The Institute of Medicine’s inclusion of “patient-centeredness” as one of the six elements of quality patient care has brought unprecedented attention to this concept as various constituencies have attempted to define, practice and measure it. Planetree, a nonprofit organization founded by a dissatisfied patient 30 years ago, has worked with hundreds of healthcare organizations to successfully operationalize whole person care in clinical settings with impressive impact on patient and staff satisfaction.

This session will present a brief history and description of the Planetree model of care, how it is used by hospitals around the country and how it has been formalized in the Patient-Centered Hospital Designation program. One of the most successful Planetree member organizations, Griffin Hospital in Derby, Conn., will be used as a detailed case study of how the model is implemented and the outcomes associated with comprehensively embracing a patient-centered approach to the patient experience. Griffin’s recent performance on HCAHPS ranked among the top 5 percent of hospitals across the nation participating in the survey program.

  1. How to identify the 10 core components of the Planetree patient-centered model of care
  2. To identify multiple strategies for improving access to information and communications between patients, families and providers of care
  3. Several ways to change the physical environment to improve patient and family satisfaction

Cancellation Policy

To cancel your webinar registration, please contact KRM Customer Service no less than 14 days prior to the program to receive a full refund. If you cancel your registration less than two weeks prior to the conference, you will receive a full credit or a refund, less a $50 processing fee. (The credit can be used toward any ACHE educational offering within one year of issue.) Once the conference begins, no credits or refunds will be granted, and all fees will be forfeited; however, you may register a substitute attendee prior to the conference if you cannot attend.
