Cancellation Policy
To cancel your participation in the seminar, please notify the Customer Service Center at least 14 business days before the start of the program for a full refund or credit, less a $50 processing fee. After that time, a refund is forfeited. For a full credit for a course cancellation or re-enrollment, please notify the Customer Service Center within 14 business days after the start of the program, less a $50 processing fee. After this time a credit or re-enrollment is forfeited. If a course includes a book, ACHE must receive notification of your cancellation prior to shipment of the book. Once the book has shipped, no refund or credit for the book will be granted. Substitute attendees are permitted. Notifications must be made in writing and can be faxed to (312) 424-9405 or mailed to the Customer Service Center, ACHE, 1 N. Franklin St., Ste. 1700, Chicago, IL 60606-3529 and must include the name of the original registrant and the new registrant's name, address and phone number.
