If your organization has a high rehospitalization rate, you might be losing potential patients to other hospitals.
In July 2009, the Centers for Medicare and Medicaid Services (CMS) started tracking rehospitalization rates for hospitals across the country and posting this information on its Hospital Compare Web site. As this data becomes increasingly available, the pressure on you to reduce these rates will intensify.
This webinar will explore how you can increase the quality of care in your organization and subsequently reduce rehospitalization rates. Evaluate how other healthcare organizations have reduced rehospitalizations by as much as 85%, and review possible strategies.
You'll Learn:
- Compare the best available rehospitalization data
- Apply strategies for reduction, including; error-free evidence-based inpatient care; transitional care between settings; high-quality proactive management in SNFs; and timely, proactive management in outpatient settings
View additional learning objectives
You'll also learn:
- Assess opportunities for improving the transition out of the hospital
- Examine why working to reduce avoidable rehospitalizations is considered a high-leverage opportunity for greater healthcare delivery system improvement
