This webinar will provide an overview of several early successful models and discuss the lessons learned from organizations that have implemented Medicare Shared Savings Programs and new population health management payment arrangements with commercial health plans. The faculty will also review several approaches to aligning the new payment models with the new care models that are being implemented in many communities, along with the challenges these approaches present. He will also explain the keys to successfully building population health models during these dynamic times.
