Journey to Improving Health Outcomes: Screening for Social Drivers of Health
To improve health outcomes and health equity, understanding what is happening in individuals' day-to-day lives outside of the healthcare they are receiving is imperative. Those circumstances, referred to as the social determinants of health (SDOH), significantly affect individual and community health and healthcare outcomes.
Former leaders from Cedars Sinai Health System will discuss the steps taken to improve the collection, sharing and use of SDOH data to comply with federal regulations that require screening and reporting on the Screening for Social Drivers of Health Measure and the Screen Positive Rate for Social Drivers of Health Measure. In addition, successful strategies and tactics implemented, how to identify opportunities and lessons learned from their efforts will be shared.
