Advancing Population Health: Securing the Right Contracts and Achieving Success
The ongoing financial challenges faced by healthcare systems, coupled with increasing numbers of physicians joining large-employed groups to sustain their livelihoods, represents an unprecedented situation. In addition, the widespread lack of affordable healthcare in numerous communities, including access to basic services, underscores the urgent need for innovative approaches to patient care beyond traditional fee-for-service models; population health offers a promising alternative.
Population health is defined as a comprehensive network of physicians, advanced providers, nurses and other healthcare professionals who collectively manage all aspects of patient health. This model involves creating large-scale care systems that address the needs of the broader population, with a particular focus on the most vulnerable groups. To ensure the success and impact of this approach, participation in the right contracts that incentivize quality care and affordability is essential. When implemented effectively, this model can drive the capital investments of tomorrow for healthcare systems, physician hospital organizations (PHOs) and independent physician associations (IPAs), reducing waste and enabling the training of future healthcare professionals to deliver care outside of traditional volume-based reimbursement structures.
Learning Objectives:
- Analyze how systems, medical groups or practices can earn more from payor agreements beyond fee schedule increases while improving quality.
- Examine strategies for incentivizing desired behaviors across the continuum of care with a focus on key areas that drive measurable impact.
- Define how managing the 'risk' associated with the lives under management can be a valuable asset and explore strategies to leverage this risk to your advantage.
