Advancing Health Equity Through Analytics and Care Coordination
The COVID-19 pandemic and social unrest have shone a spotlight on health disparities that have long existed. South Carolina is not a Medicaid expansion state, and it has a significant volume of patients who are uninsured. More than 20% of residents in Berkeley, Charleston and Dorchester counties (within the Roper St. Francis Healthcare service area) are without health insurance at any given time, and nearly 16% of adults do not have a regular provider. Join this webinar to learn more about how RSFH recognized the need for tools and processes to proactively identify underserved populations and deploy proactive intervention and management across the care continuum, looking beyond just clinical data indicators. Participants will also learn about how these efforts have translated into reductions in racial and socioeconomic disparities among RSFH’s patient population.
