Hospitals face complicated and continually changing Medicare rules, regulations and guidelines. The Centers for Medicare & Medicaid Services’ 2014 Hospital Inpatient Prospective Payment Systems for Acute Care Hospitals includes new requirements for classifying patients as inpatient vs. outpatient—now known as the two-midnight rule. It also includes new conditions of payment that include written physician certification as well as requirements for admitting physician qualifications, order timing, administrative documentation and clinical documentation.
