The Power of Genomics: A Population Health Strategy

Elegant in its simplicity, the premise behind the use of genomics in population health management packs a powerful punch: The more information you have about a patient population at the molecular or genetic level, the better prepared you are to predict, prevent, diagnose and treat disease. Seeing this potential, more health systems are investing in genomics to improve community health by identifying individuals at risk of actionable conditions. We spoke with some of the healthcare organizations that are providing or planning to provide genetic or genomic testing for the cover story of the latest issue of Healthcare Executive to learn more about what they hope to achieve with this technology.

CMS Proposes Changes to Close Health Equity Gaps in ESRD PPS

The Centers for Medicare & Medicaid Services recently proposed changes to reduce healthcare disparities among patients with chronic kidney disease and end-stage renal disease in its ESRD Prospective Payment System annual rulemaking. The proposed rule would update the ESRD PPS payment rates, make changes to the ESRD Quality Incentive Program and modify the ESRD Treatment Choices Model. The proposed changes to the ETC Model policies would aim to encourage dialysis providers to decrease disparities in rates of home dialysis and kidney transplants among ESRD patients with lower socioeconomic status, making the model the agency’s first CMS Innovation Center model to directly address health equity.

Disadvantaged Medicare patients suffer from ESRD at higher rates, according to studies on racial, ethnic and socioeconomic factors conducted by the CMS Office of Minority Health. They are also more likely to experience higher hospital readmissions and costs, and receive in-center hemodialysis because their kidneys are no longer able to perform their function. Studies also indicate nonwhite ESRD patients are less likely to receive pre-ESRD kidney care, and become waitlisted for a transplant or receive a kidney transplant.

The proposed changes to the ETC Model include testing a new healthcare approach that rewards ESRD facilities and managing clinicians participating in the model for achieving significant improvement in the rates of home dialysis and kidney transplants for lower-income beneficiaries. If finalized, these changes would take effect Jan. 1, 2022.

For more information on CMS’ recently proposed changes to the ESRD PPS, you can read the proposed rule here, and a fact sheet about the changes here.
