Collaboration Helps Solve Population Health Challenges

Patrick Rohan, FACHE, CEO of Good Samaritan Hospital, San Jose, Calif., says part of a healthcare leader’s job is recognizing that they can improve lives outside of the four walls of the hospital. “To change the landscape, we must take a holistic view of what it means to care for our patients and recognize that treatment extends beyond surgical procedures or medications.”

In the cover story “Strengthening Communities Through Health Outcomes” in the current issue of Healthcare Executive, Good Samaritan and Advocate Trinity Hospital, Chicago, have shown how collaborating with their community results in innovative, widespread and long-lasting solutions to pervasive population health challenges. These include investing in new neighborhood care sites, upgrading facilities, creating a food pharmacy and addressing affordable housing.

“We have the data to show the status quo isn’t working,” says Michelle Y. Blakely, PhD, FACHE, president, Advocate Trinity Hospital. Instead of leaving the market, however, Advocate Trinity is “doubling down and disrupting the idea that closing the gap is too difficult.”

Continued engagement will create sustainable results. If the approach is working, it gets replicated, Rohan says. “If it isn’t, we pivot and keep searching for a better solution.”
