Developing Strategies to Mitigate Diagnostic Error 

Diagnostic errors cause more morbidity and mortality than all other patient safety problems combined. Patient safety experts Paul L. Epner, MEd, CEO and co-founder of SIDM, and Sue Sheridan, DHL, director of patient engagement, SIDM, will discuss with webinar participants the impact this issue is having on patients and system finances. By the end of the webinar, attendees will be able to evaluate the key approaches organizations can take to mitigate diagnostic errors.

In June 2019, ACHE joined the Coalition to Improve Diagnosis, led by SIDM. The coalition, which includes more than 50 healthcare and patient advocacy organizations, will serve as an extension of ACHE’s “Leading for Safety” initiative.

Aspiring to Zero Patient Harm

“Historically, analysis of the causes of serious safety events in healthcare has focused on active errors resulting from human performance,” writes Charles A. Mowll, LFACHE, president and CEO, Patient Safety Coaches Academy, in his new book, Five Disciplines for Zero Patient Harm: How High Reliability Happens. “A more sophisticated and comprehensive approach to understanding the causes of accidents or serious safety events in high-reliability organizations involves the analysis of the three factors that contribute to safety events: human performance, system performance and defense and controls performance.”

This passage is one example of the real-world, how-to guidance Mowll offers for driving fundamental change that consistently achieves safe patient care. His book is one of several resources ACHE provides to leaders of hospitals and healthcare systems that are seeking to reset their patient safety goal to zero patient harm.
