Post-acute care is one of the fastest growing segments of U.S. healthcare. Every day, 93 million people are discharged from U.S. hospitals, and at least half of these patients require additional post-acute care before they can return home independently. Health reform is likely to transform how health systems manage episodes of care, presenting both a leadership challenge and an opportunity.
This webinar provides an overview of the post-acute care continuum, focusing on physicians’ and health professionals’ role in care management, the regulatory and payment environment, and the administrative challenges that appear when there are gaps in health service delivery. You’ll also see the importance of managing Medicare “days” for patients who will move to post-acute care providers and why silos in care management hinder effective and seamless transition.
