Moving Toward Population Health Management

Substantial health disparities have been uncovered in the wake of the COVID-19 pandemic, including the looming Medicare insolvency crisis due to the aging population. In light of this dilemma, experts argue there has never been a more compelling time to adopt a system of care based on population health management. In Chapter 3 of his book, Caring for Our Communities: A Blueprint for Better Outcomes in Population Health, author Mark Angelo, MD, shares the expertise he has acquired as a senior administrator and provides tactical guidance for developing effective population health programs. He answers many questions about value-based care models, uses real-world examples and shares perspective from industry experts to clarify the underlying concepts of value-based initiatives.

Caring for Our Communities covers several topics of value-based, population health-driven care, including the “payvider” model and how it breaks down the silos between payers and providers.

This book provides a guide for creating an equitable, outcomes-focused system, using the most efficient resources to maximize health outcomes. Chapter 3, titled “Community Wellness: The Promise of More Equitable Outcomes,” is free of charge to ACHE members. Among topics the chapter explores are the obstacles to health, social drivers of health, the importance of addressing social factors impacting health and the continuum of care.

Congress 2024: Expert Insights Into Hot-Topic Issues Important to You

At ACHE’s Congress on Healthcare Leadership, Monday, March 25, to Thursday, March 28, you will learn from experts within healthcare and beyond about critical issues facing the field. Following are previews of sessions that will address hot topics such as implementing AI, healthcare disruptors, health policy and politics, optimism in the future and transformative change.

  • Join Harry S. Saag, MD, FACP, national medical director, virtual care, Walgreens Health, and Kameron Matthews, MD, JD, FAAFP, chief health officer, Cityblock Health, for “Hot Topic: How Disruptors Are Transforming Healthcare Access and Affordability.” During their presentation, Saag and Matthews will examine approaches healthcare disruptors are taking to meet the needs of healthcare consumers.
  • Join American Hospital Association executives Richard J. Pollack, president/CEO, and Stacey Hughes, executive vice president, government relations and public policy, for a deep dive into the intricate and shifting world of healthcare policy and politics during “Hot Topic: AHA Policy and Politics Update.”
  • Zachary Karabell, an authority on future trends, geopolitics and the changing global economic system/author of The Leading Indicators and Inside Money, will present “Hot Topic: The Case for (Edgy) Optimism.” During this session, Karabell makes the case for optimism—in terms of politics, economics and culture—in that it is a vital ingredient in recognizing the opportunities that abound and seizing them.

Redeem Leader-to-Leader Rewards Points by Dec. 31

Members have until Dec. 31 to redeem any points earned in 2022 when they refer colleagues to join ACHE through the Leader-to-Leader Rewards Program

The Leader-to-Leader Rewards Program recognizes ACHE members with reward points when they recruit colleagues and encourage current Members to advance to Fellow. Members earn reward points for referring new members to ACHE, but only for applications received through Dec. 31. Reward points can be redeemed as soon as they are earned and up to Dec. 31 in the year after points were earned. Points can be redeemed for $25 discount coupons or ACHE merchandise, such as a UV phone sanitizer with a wireless charging pad, ACHE face masks and more.

Students and Host Sites: Apply to IFDHE’s Summer Enrichment Program

The Institute for Diversity and Health Equity’s Summer Enrichment Program is now open. The SEP is a 10-week, paid internship for graduate students who are currently pursuing an advanced degree within healthcare or a related field. Students in the program are paired with a healthcare organization, where they can benefit from experiential learning, mentorship and networking. Students from underrepresented backgrounds are strongly encouraged to apply.

Additionally, healthcare organizations that are committed to diversity, equity and inclusion are strongly encouraged to apply to serve as a host site. Host sites will provide internship opportunities for graduate students in the field to strengthen the pipeline and build the next generation of diverse executives and leaders in healthcare. Students will gain experience that will equip them to advance health equity, DEI and justice in healthcare.
